Extreme doesn't do a great job of responding to complaints.

Please note that the US outfit looks completely unrelated to the Canadian company, as far as I can research. However, there are a number of complaints filed at the Better Business Bureau towards multiple locations in the GTA (listed on your link).

This is an excellent reminder. File a complaint first with Extreme management, and then if it is unresolved, file a complaint against them at the BBB. I for one often refer to the BBB website before joining a gym, or to find out about a company. This looks very serious to me.
